Parents please be advised that all students with life threatening health conditions must have a doctor signed emergency care plan on file for the current school year. These emergency care plans must be on file with the school nurse in order for your child to attend classes. Students requiring medication to be administered at school must also have a doctor signed form on file with the school nurse. You can download all necessary forms, as well as, immunization requirements below.
